Hypertension Clinic with Data Registry System

The PSH Hypertension Clinic is a patient-centered initiative of the Philippine Society of Hypertension (PSH) designed to improve the detection, diagnosis, treatment, and long-term control of hypertension through standardized, guideline-directed care. Integrated with the PSH Data Registry System, the program will systematically collect, monitor, and analyze patient data to evaluate treatment outcomes, medication adherence, blood pressure control, and the sustainability of hypertension management.

The data registry will support evidence-based clinical decisions, track patient progress, and measure key performance indicators, including newly diagnosed cases, validated office and home blood pressure measurements, achievement of target blood pressure, and sustained control at 6, 12, and 24 months. Through digital documentation, continuous monitoring, and data-driven reporting, the initiative aims to strengthen hypertension care delivery, improve patient outcomes, and generate reliable evidence to guide national hypertension programs and policies.
